There are requirement for the fact that a person is completely detached from the sense of reality. The person requires becoming completely un-empathetic. Though, in some cases empathy can also be present but the person does not feel any guilt. There are several aspects of culture which leads to different cases where we see that there are several cases where a person never reaches the state of guilt.
